Enrollment Information for Jemez Day School
We're thrilled to welcome new students into our community. Here’s what you need to know to enroll your child:
Open Enrollment
Jemez Day School has an open enrollment policy, welcoming students throughout the year.
Kindergarten Enrollment
Incoming kindergarten students must be 5 years of age by September 1.
Immunization Requirements
Immunization records are required for all kindergarten and newly enrolled students. Parents are expected to submit information at the time of enrollment. Students lacking up-to-date immunizations will be notified.
Required Registration Documents
To be considered for admission, the parent/guardian must provide the following registration documents:
- Completed enrollment forms
- Enrollment forms can be picked up at the school office.
- Birth certificate
- Immunization record:
- DPT/DT, Polio, Rubella (Measles), Mumps Rubella (German Measles), and Hepatitis Series.
- Certificate of Indian Blood
- Physical examination
- Required for kindergarten students, newly enrolled students and athletes.
- Home language questionnaire
- Official transfer records
- For new student enrollment, a transcript from the previous school and updated medical information are required.
- Medical/consent form for medical services
- Student emergency contact and bus information form
